COVID-19 VACCINES
At Dr. Yordan Offices,
we encourage our patients to schedule an appointment to have a discussion with your provider about you and the safety of the vaccine for you.
NEW PATIENTS
Who can get the COVID-19 Vaccine?
In accordance with Executive Order 20-315 issued on December 23, 2020, providers shall only administer COVID-19 vaccine to the following populations: long-term care facility residents and staff, persons 65 years of age and older, and health care personnel with direct patient contact. Hospital providers, however, also may vaccinate persons who they deem to be extremely vulnerable to COVID-19. As additional information becomes available, it will be posted on the Florida Department of Health’s COVID-19 website.

Please arrive 30 minutes before your arrival time. Packet for Patients that are establishing in our practice. Please bring the packet filled out and arrive 30 minutes before appointment time. Click here to complete your new patient intake form online..
To ensure that you are seen, please be sure to bring the following to your appointment:
- Original insurance cards (Photo copies will not be accepted)
- A valid government issued photo ID
- A current medication list that includes both dosage and frequency
- Your completed new patient packet.
- For safety purposes all minor children under 18 years of age, must be accompanied by a parent or legal guardian.

The Patient Portal enables our patients to communicate with our doctors, nurses, and staff members easily, safely, and securely via the Internet using a browser or a smart phone using an application called Healow that is a free download. If you sign up you will be given your ID and password, enabling you to access the Portal and your records, including lab and diagnostic test results, educational information, billing statements, and other health information. The Healow app is the mobile app that allows you all the same access as the portal with your mobile device.
PROVIDERS
I am a national board certified advance practice registered nurse with the American Academy of Nurse Practitioners (AANP) with a specialty in Adult-gerontology. I completed my Bachelor of Science Degree in Nursing from Regis University, Denver, CO and then earned a Master of Science in Nursing from South University, Savannah, GA.
I have had a love for the geriatric population since I was a child. My goal is to educate my Patients in management of chronic diseases, as well as, provide education for prevention of further disease processes. It is important to get to the root of the problem and not treat just the symptoms.
I currently reside in Ocala with my husband of 36 years. We have 3 married adult children which have blessed us with 6 grandchildren ages 14 to 3. We enjoy spending time with our family. I like to do a multitude of crafting, collecting Dept. 56 and Nutcrackers, as well as traveling.

Dr. Juan Yordán, MD is a Family Medicine Specialists in Lady Lake, FL and has been practicing for 20 years.
Dr. Yordán was born in Ponce, Puerto Rico. He earned his Bachelors degree of Science at Catholic University of Puerto Rico and his Medicine Degree at Ponce School of Medicine; Dr. Yordan did his three year Family Medicine residency at Dr. Pila Hospital.
Dr. Yordán is married to his wife Ninette and has two children; Natalia and Juan Carlos Jr.
In 2004, he moved from Puerto Rico to Virginia where he worked for three years at Norton Community Hospital, as the attending physician and emergency room physician.
In 2007, He decided to move to Ocala, FL where he worked six years for Family Care Specialists; then he opened his own practice in The Villages.
Dr. Yordán has been involved in teaching since 1998 and was awarded the Degree of Fellow From The American Academy of Family Physicians in 2016.
